Dear Camp Quality Campers and Parents,
There are only a few months left before we get to enjoy another great week at camp! CQH 2011 will begin on Wednesday June 8th and end on Sunday June 12th at 12:00 PM, so get ready to embark on a Time Traveling Adventure! If you would like to attend, please send your completed applications to the address below. Following are some instructions regarding the applications – please read them carefully.
Camp will once again be located at the Gene Eppley Campground south of Bellevue. Their website is www.geneeppleycamp.org.
Please note that the deadline for applications is April 25, 2011. The links to the applications are at the end of this paragraph. Due to camp growth and our 1:1 camper/companion ratio, we will no longer be able to accept applications after this date, with no exceptions. We are also unable to accept telephone or email “RSVPs” – all camp paperwork, with the exception of the physician signed medical form, must be received by April 25th. We appreciate your help in streamlining our registration process! Upon receiving your registration forms, I will send an acknowledgement email or postcard indicating that your forms have been received. Directions to the campground, a supply list, and final registration information will be mailed to you three weeks prior to camp.

Campers with cancer/cancer survivors will need a medical form completed by their health care provider. This form is page 5 of the camper application and may be sent after the application deadline, but must be received by June 1st. Please note that camper packets are not interchangeable. Please include a current copy of your insurance card with your application.
Our use of the CampDoc online medication administration/documentation system was very successful last year, so we will be using the program again this year. All parents (including those whose children do not take medications) will need to register online and enter allergy history/medication information prior to camp. Please include a valid email address that you check regularly on your children’s applications. We will use this email address to ensure that you have access to the CampDoc site and will notify you when the site is available for 2011 info. Since we are using CampDoc, please disregard the last page of the application, the Medication Form.
Finally, and much more fun than paperwork – the dance theme this year will be “The Jungle”, so dust off your cheetah prints and your panama hats!
